Create a final report in response to the VOC report found in the course. Complete the “Final Report Template,” by doing the following:

Part 1:

A.  As part of the control phase of the improvement project, create a process control plan by doing the following:

1.  Define measures or key indicators that will be used to track progress.

2.  Identify the threshold for the measures or key indicators that will signal a need for a potential response plan.

3.  Explain your potential response plan if a key indicator fails to meet the standards established in parts A1 and A2.

B.  Identify five individuals who will receive the reports of the results of the quality improvement project, including the following information for each individual:

Part 2:

E.  Identify three success factors that are critical in any quality improvement initiative or effort.

F.  Evaluate the effectiveness of the DMAIC model by doing the following:

1.  Explain how the DMAIC model could be applied to address the three key success factors from part E in the current project plan at Shelbyville Medical Center.

2.  Discuss the appropriateness of the DMAIC model as a framework for improvement to address the defined project aim.

3.  Discuss an alternate improvement framework or method that might be more or equally applicable in managing problem resolution and organizational change at Shelbyville Medical Center.
