Model Selection

Choose one of the following from the QI options.QI Models: PDSA, FADE, or DMAIC (as part of Six Sigma)Provide a clearly designated title for your QI model that reflects the model selection and project focus. Completing your selected Model

Each model has different components that should be completed. You are responsible for completing those components of your selected model based on your POI. You are not required to use paragraphs or complete sentences on the model, but it should be complete, legible, and logical to the reader.Example 1:-if a section of your model requires you to add the literature used to support your POI, then you should include the titles of the Guideline, Meta-analysis, single studies, etc. used in your prior assignments.

Model Development

You are addressing the additional set of questions/criterion. This must be written using the Appendix Template provided to you in the directions. Do not alter the template.1. Specificity: define what you are going to do by including your clinical question must be clearly identified in PICO format. (1 point)2. Measurable: Include 2-3 measures that you will use to determine successful implementation. Measures must include: who, what, when, and how often. Metrics or benchmarks should be used to indicate the standard by which you know the measure has been met, i.e. 20%, 90% pass rate, etc. (2 points)3. Achieve: What skills and resources are needed? Include skills of your team members (stakeholders) and resources needed such as technology, financial, education, etc. A minimum of 2 skills and 2 resources should be documented and explained how they will meet the needs of the QI model development plan. (2 points)4. Relevant: Does this project align with the systems’ mission/vision? To successfully demonstrate this, you must state how the mission or vision meets the implementation plan. You may need to include the actual mission or vision statement in this step. (1 point)5.Time: include a time frame for all steps of the model from planning to implementation and evaluation. (2 points)

Strengths and Barriers

Include the strengths and barriers to implementation. Answers may be in short, bullet format and should be included in the appendix template provided. A minimum of 3 strengths and 3 barriers should be included.
