Scenario
For this assignment, you are creating an infographic to share with employees across a large physician group organization, which consists of licensed health care professionals and administrative staff of approximately 500 to 600 people. The infographic you create will inform the patients and care providers about various HEDIS measures, how the physician will apply the measures to the patient’s health care, how the measures improve patient health outcomes, and what steps physicians must follow to ensure the measure is met now and in the future. The infographic will be placed on the walls of exam rooms at the organization’s facilities.

- To view each HEDIS measure, you need to expand each topic (Effectiveness of Care, Access and Availability of Care, Utilization, Risk Adjusted Utilization, and Measures Reported Using Electronic Clinical Data Systems) to see the list associated with each domain.
Select one measure and review the specific details by selecting the hyperlink from the list.
To create the infographic, you may use any infographic tool in the Digital Toolbox. You must be able to download and submit the final product in Waypoint.
In your infographic:
Identify the risk factors related to being part of a marginalized group that increases the probability of poor health outcomes if the patient does not comply (e.g., race/ethnicity, socioeconomic status, obesity, LGBTQ+, etc.).Consider patients who are likely to experience inequity related to the selected measure. Your infographic should be pleasing to the eye and professional-looking, and it should consider your audience: the patients and the staff members working with them. Thorough details are required, but your infographic should not be too busy.
