    Scenario

For this assignment, you are creating an infographic to share with employees across a large physician group organization, which consists of licensed health care professionals and administrative staff of approximately 500 to 600 people. The infographic you create will inform the patients and care providers about various HEDIS measures, how the physician will apply the measures to the patient’s health care, how the measures improve patient health outcomes, and what steps physicians must follow to ensure the measure is met now and in the future. The infographic will be placed on the walls of exam rooms at the organization’s facilities.

    Directions

Choose one of the HEDIS measures from the domains shown on the NCQA

    HEDIS Measures and Technical Resources

    webpage.

To view each HEDIS measure, you need to expand each topic (Effectiveness of Care, Access and Availability of Care, Utilization, Risk Adjusted Utilization, and Measures Reported Using Electronic Clinical Data Systems) to see the list associated with each domain.

Select one measure and review the specific details by selecting the hyperlink from the list.

To create the infographic, you may use any infographic tool in the Digital Toolbox. You must be able to download and submit the final product in Waypoint.

    In your infographic:

  • Identify the HEDIS measure as your title.
  • Explain how the measure improves patient health outcomes.
  • Identify the patient eligibility criteria (e.g., Breast screening is a measure that assesses women ages 50–74.).
  • List two data elements that must be collected from the patient’s Electronic Health Record (EHR) or claims data.
  • State two actions the organization must take to fulfill the measure.
  • Describe the patient’s responsibility in complying with the measure (e.g., one thing patients must do).
  • List at least one follow-up process for the measure if the patient does not comply.
  • Identify the risk factors related to being part of a marginalized group that increases the probability of poor health outcomes if the patient does not comply (e.g., race/ethnicity, socioeconomic status, obesity, LGBTQ+, etc.).Consider patients who are likely to experience inequity related to the selected measure. Your infographic should be pleasing to the eye and professional-looking, and it should consider your audience: the patients and the staff members working with them. Thorough details are required, but your infographic should not be too busy. Your infographic should use the following:Appropriate font size and style (Use a San serif font, such as Calibri or Arial.)Appropriate color contrast (Use a dark background with light-color text or a light background with dark-color text.)Images (required) You may use any of the following resources to locate copyright-free and no-cost images or use the INSERT > PICTURES > ONLINE PICTURES function in your document.Affect: Disabled and Here Collection MorgueFileNappyPexelsPixabayUnsplashWikimedia Commons Searching and Downloading Images from Wikimedia Commons (videoThe HEDIS Measure Infographicmust contain the components outlined in the directions.must contain images.must contain at least 700 words and be at least three pages in length, not including references.must be written in your own words (i.e., no content may be copied from other sources). must use proper grammar and spelling.must use a minimum of two credible sources to support your work. The Scholarly, Peer-Reviewed, and Other Credible Sources table offers additional guidance on appropriate source types.
